SUPPORTED LIVING (18+)
We are here to help you cope if you decide to live at home instead of residential care home.
We can support you
with the following services:

- Taking medication
- Maintaining the home
- Paying bills
- Budgeting
- Maintaining tenancies
- Employment
- Further education
- Learning new skills
- Becoming more confident
- Participating in community activities
- Maintaining a good social life
- 24 hour support is available if needed
THE RESIDENT PROFILE IS:
- Men and women
- 18 years + of age
- Primary diagnosis of Mental health
- Dual diagnosis Learning disability
- May have complex needs
- May have a forensic history
- May be subject to a Community Treatment Order (CTO)
EXCLUSION CRITERIA :
- Under 18 years of age
- Detained under the Mental Health Act
- Detained under Deprivation of Liberties (DOLs)
- Schedule 1 offenders (any sexual offences)
- History of arson
- Substance abuse (alcohol, drugs)
- Nursing needs
- Mobility issues (stairs)
LIVE IN CARE
We take a personally tailored approach to live-in care. Each of our Service Users are unique and so our tailored programs ensure that everyone’s individuality is taken into account, including their families, friends and lifestyles.
All of our Carers are highly trained and the care plan implemented is tailored to you or your loved one. We work with our Service Users, their families, their GPs, hospitals and other organisations to create an appropriate and respectful level of live-in support.
TRANSITION SUPPORT
We help young adults moving from children’s services to adult services to develop their independence and have choice and control over the support they need to achieve their hopes and ambitions. The person is at the core of the service we provide so we plan services around each individual person and their needs.
Transition Support Service includes:-
- Assistance with finding appropriate accommodation that meets the needs of each individual.
- Skills acquisition and development to enable the adult become independent.
- Assisting the young person to engage with their local community and enjoy all it has to offer.
- Encourage people to enjoy family life, make friends and lead a full life.
- Providing person centred support, working in partnership with individuals, their families and healthcare professionals.
- Working with the individual and their families to select their own support staff.
- Supporting the young person to find employment
- Help individuals approaching the age of 18 who has been assessed as requiring care and who would like us to start planning their future support.
DOMICILIARY CARE
We provide domiciliary care aimed to provide a high standard of personal and domestic care to older people, with a disability who may be suffering with a long term illness and will require support to live in their own homes.
RECRUITMENT
We are always seeking hardworking and quality Registered Nurses and Nursing Assistants to work a large number of ongoing shifts with our clients all over the United Kingdom (UK).
